Item Outcome of Treatment of Clubfoot at the Korle Bu Orthopaedic Unit Using the Ponseti Method(2013-12-09) Bandoh, A.K.; Addo, A.O.; Segbefia, M.Clubfoot deformity is common in Ghana and the understanding of the deformity is woefully inadequate among the general population. The current approach in the treatment of idiopathic clubfoot has focused on conservative techniques. The Ponseti technique has superior outcomes. This involves weekly manipulations and casting of the clubfoot to gradually correct the clubfoot. The functional result is good with the painfree and plantigrade foot after treatment very acceptable to the parents. This charpter addresses the predictiveness of the Pirani score in determining the number of casts in the treatment of Clubfoot at Korle-Bu Orthopaedic Unit using the Ponseti method. Each clubfoot was evaluated using the Pirani score at two weekly intervals with scores documented. Abduction brace was applied at full correction until age four years. There were 74 respondents made of 31 (42%) females and 43 (58%) males. The highest score at the beginning of treatment was 5 and at the end of treatment it dropped to zero in some cases. Overall mean score at 6 weeks was 2.5, at 10 weeks was 2.1 and at 16 weeks was 1.9. The highest drop in score occurred within the first 6 weeks. The Ponseti method is an effective method of treating idiopathic clubfeet. When started early, its outcome can be predicted with the Pirani score. Most of the correction took place in the first six weeks of casting. Introduction Clubfoot is a complex developmental deformation of the foot and the genes responsible for the deformity has been reported to be active from the 12th to the 20th week of foetal life and can last until three to five years of age. 1 The deformity has four components; equinus at the ankle, varus at the hindfoot, forefoot adductus and cavus. The goal of treatment is to attain a functional, pain-free, plantigrade foot with good mobility. In the developing world, many of the cases are untreated or poorly treated, leading to a neglected clubfoot. These children undergo extensive corrective surgery later, often with disturbing failures and complications.The foot after surgery looks better, but is stiff, weak and often painful. Clubfoot in an otherwise normal child can be corrected in two months or less with the Ponseti method of manipulations and plaster cast application with minimal or no surgery. The Ponseti method of correction of clubfoot deformity requires serial corrective casts with long-term brace maintainance of correction. Treatment needs to be started as soon as possible and should be followed under close supervision. 2 Brace should be worn full-time (day and night) for the first three months after the tenotomy cast is removed, then for 12 hours at night and 2 to 4 hours in the middle of the day, for a total of 14-16 hours (night and naps protocol) during each 24-hour period. In the brace, the knees are left free. Bracing is continued for up to four years of age and the outcomes are good. This method is suited for developing countries with scarce resources and can be learnt by allied health professionals very easily.4 The outcomes can be evaluated using the Pirani score which is easy to use with a good interobserver and intraobserver reliability5 Considering that about 100,000 babies are born worldwide each year, with 80 percent with clubfoot occurring in developing nations,4 it is important for many more people to learn the Ponseti method. If the clubfoot is not treated, it leads to severe disability where shoe wearing and mobilization are greatly impaired. The deformity associated with the neglected clubfoot is not acceptable and the children often end up receiving extensive surgical procedures resulting in a chronically painful foot. The current method of early treatment in Ghana is the Ponseti method. Literature Review Clubfoot is the commonest congenital deformity in babies. More than 100,000 babies are born worldwide each year with congenital clubfoot. Around 80 percent of the cases occur in developing nations.4 The male-to-female ratio is high at 3:1.5 Palmer6 explained this by suggesting that females require a greater number of predisposing factors than males to produce a clubfoot deformity. Social bias and increased attention towards males in our region can account for the higher incidence in males. The order of birth also seems to have an influence on the occurrence of clubfoot, with 65percent of cases in •196• Chapter 16 first-born children, which is in accordance with various studies.7 There is no relationship of clubfoot to the type of birth.4 Many different clubfoot classification systems have been proposed but no single one is universally accepted. At a minimum, a useful classification should distinguish postural, self-resolving clubfoot from true clubfoot which requires orthopaedic treatment. A more advanced classification could discern subtypes of nonsyndromic clubfeet such as complex idiopathic or neurogenic-type clubfeet.8 An ideal classi- fication would accurately portray the initial severity of the deformity and have prognostic ability.9 The Pirani score is easy...Item Predictors of hip fracture mortality in Ghana: a single-center prospective study(Archives of Osteoporosis, 2021) Baidoo, P.K.; Odei, J.B.; Ansu, V.; Segbefia, M.; Holdbrook-Smith, H.Summary To determine risk factors influencing mortality in patients with proximal femur fractures in a Ghanaian hospital over a 4-year period. Methods Incidence of mortality was assessed among 76 participants with proximal femur fractures from January to December 2014 and followed up for 4 years. Outcomes of interest were mortality at 1 month, 6 months, 1 year, and 4 years. Hazard ratios (HRs) were calculated using Cox proportional hazards regression, adjusting for mortality risk factors. Results Among the 76 participants (mean age 75.8 years [SD = 12.02], 36 (47.4%) males), there were 21 death cases. The mean time of injury to surgery was 16.4 (SD = 16.2) days. Hip fractures comprised of 38 (50%) intertrochanteric, 35 (46.05%) transcervical, and 3 (3.95%) basicervical. Mortality at 1 month, 6 months, 1 year, and 4 years were 6.6%, 13.2%, 19.7%, and 27.6%, respectively. Multiple regression analysis showed a yearly increase in age that was associated with a 1.03-fold increase in the risk of death (p = 0.17). Comparing males to females, there was a significant difference in mortality (HR = 5.24, p = 0.03). Participants with basicervical hip fracture versus those with transcervical hip fracture were at higher risk of dying (HR = 28.88, p = 0.01). Patients with abnormal/low creatinine as compared to those with normal creatinine were at higher risk of dying (HR = 5.64, p = 0.005). Also, participants with an American Society of Anesthesiologists (ASA) score of III or IV were 2.73 times more likely to experience death than those with an ASA score of I or II (95% CI: 0.93–8.89, p = 0.08). Additionally, a higher risk of death was associatedwith patients with chronic obstructive pulmonary disease (COPD) (HR = 53.45, p = 0.001) and osteoporosis (HR = 8.75, p = 0.006). Conclusion Being male, having basicervical hip fracture, abnormal/low creatinine, and a history of COPD and osteoporosis were the main predictors of mortality in the study population. They occur in various types and forms de pending on the types and numbers of bones involved at the elbow. CHRS may present with elbow deformity and limitation of function. Appropriate timely diagnosis and counseling are required since CHRS is mostly managed conservatively according to literature and may prevent avoidable fractures of the radius from attempts by parents to straighten the flexed fixed elbow and finally offer adequate time for delayed surgical intervention which is usu ally ineffective and unhelpful.
