Item Aorto-oesophageal fistula: An unusual cause of upper gastrointestinal bleeding(Ghana Medical Journal, 2002-12) Clegg-Lamptey, J.N.A.; Tettey, Y.; Hodasi, W.M.; Amenuveve, C.We report a case of a 35-year-old man who pre- sented with haematemesis, chest pain and a very high fever following the swallowing of a fishbone five days previously. He was correctly diagnosed as having a mediastinitis but had an exsanguinating haematemesis and died within 24 hours of arrival in hospital. Post mortem revealed mediasinitis and an aorto-oesophageal fistula. The difficulty in making the diagnosis of this condition and the often- resulting catastrophic haemorrhage that occurs are discussed. It is a condition that should be considered in a patient presenting with fresh haematemesis and a history of ingestion of a foreign body five to seven days previouslyItem A case of iliac artery aneurysm arising from herniorrhaphy and its management(Ghana Medical Journal, 1990-06) Hodasi, W.M.Complications of herniorrhaphy are many but aneurysm of the artery whether iliac or femoral is very rare.it is still possible due to the repair of the posterior wall or any other iatrogenic condition. Unlike an aneurysm due to arteriosclerosis, the high rate of injection complicates the therapy so that normally an extra-anatomic bypass is indicated . In the absence of a bypass. adequate collaterals could be a substitute until the septic wound heals completely. This is shown in the changes of the- ankle pressure indices and the claudication distance which increases gradually after closure of the femoral artery. indicating the change in its haemodynamics. Definitive treatment can therefore be delayed for a period of at least one year in a young man till the wound heating is complete as there is a chance of collateral formation. It is known that haemorrhage. infection and recurrence are among the common complications of an inguinal hernia repair. Aneurysm of the iliac artery is to be expected. following trauma to the artery. Such a false aneurysm could originate from Posterior wall repair and can later be mistaken for a haematoma. Traumatic aneurysm of the iliac artery has not been reported in Ghana. This paper reports of an aneurysm of the right external iliac artery which was detected 4 months after a right-sided herniorrhaphy.Item Comparison of four-day and ten-day post-mastectomy passive drainage in Accra, Ghana(East African Medical Journal 84(12): 33-37, 2007) Clegg-Lamptey, J.N.A.; Dakubo, J.C.B.; Hodasi, W.M.Item lIeosigmoid Knot; A case report(Ghana Medical Journal, 1987-09) Hodasi, W.M.A case of an ileo-sigmoid knot which presented with a sudden onset of colicky abdominal pains. is presented. Unlike a sigmoid volvulus, the abdomen was moderately distended and the pain became continuous and increased in intensity. This is a condition whereby the ileum is twisted along its own axis and encircled by the sigmoid colon. It occurs predominantly in males patients and predisposing factors could be due to nutritional conditions and tribal tendencies. It is difficult to recognise it clinically and at operation, the surgeon may be able to identify it if the gut is not destroyed. The ileum is mostly gangrenous and demands resection and a continuous or an ileo-colic anastomosis.
