Antibody levels to the virus capsid antigen of the Epstein-Barr virus and to the brain cell antigens myelin and cerebroside were elevated during active CNS disease. Immune complexes were present in levels above 100 micrograms/ml in most patients with CNS involvement but tended to be low or negative in patients without CNS disease. Oligoclonal IgG bands were present in 12 of 13 patients with CNS disease and in only 3 of 26 patients with no clinical evidence of disease. None of these markers were present in 6 other tumor patients without CNS disease. The presence of these markers in 12 of 13 patients in whom CNS disease was involved suggests that these markers may be useful in determining the status of the tumor with regard to involvement of the CNS.Item Combination chemotherapy in abdominal Burkitt's lymphoma.(Cancer, 1977) Nkrumah, F.K.; Perkins, I.V.; Biggar, R.J.In a clinical trial, 42 patients with abdominal Burkitt's lymphoma (BL) were treated with a combination regimen, code-named CVA, consisting of cyclophosphamide (CTX), vincristine, and cystosine arabinoside. In addition, intrathecal methotrexate (i.t. MTX) was administered as prophylaxis against subsequent central nervous system (CNS) involvement. Induced remissions, relapse, and survival were compared with those in a preceding group of 44 patients with abdominal BL treated with CTX along. Remission rate did not differ significantly in the two treatment groups, although induced remissions were higher in the CVA plus i.t. MTX-treated group (94% vs. 83%). Remission duration was significantly increases (p less than .05) and CNS relapse significantly reduced (p less than .05) in the group treated with CVA and i.t. MTX. The combination therapy was associated with higher early deaths during treatment, which adversely affected the overally survival. It is suggested that a reduction of the initial chemotherapeutic doses, particularly for patients with extensive tumor load, could further improve on the results of this trial.Item Specific transfer factor with activity against epstein-barr virus reduces late relapse in endemic Burkitt's lymphoma(Anticancer Research, 1990) Neequaye, J.; Viza, D.; Pizza, G.; Levine, P.H.; De Vinci, C.; Ablashi, D.V.; Biggar, R.J.; Nkrumah, F.K.Twenty-seven children with abdominal Burkitt's lymphoma (stage III), who had achieved complete remission, were entered into a prospective controlled trial of adjunct treatment with Epstein-Barr virus (EBV)-specific transfer factor (TF). Two patients treated with TF and 2 controls relapsed early (? 12 weeks). Two out of 12 TF-treated patients and 5 out of 11 controls subsequently suffered relapses. Time to first late relapse was longer among TF-treated patients (p = 0.08), and no late relapse occurred while a patient was receiving TF treatment. Thus it seems that specific TF might be useful in the management of endemic Burkitt's lymphoma and also in the treatment of other virus-associated cancers and diseases.Item HLA-DR7 association with African Burkitt's lymphoma.(Human Immunology, 1985) Jones, E.H.; Biggar, R.J.; Nkrumah, F.K.; Lawler, S.D.Association between HLA-DR7 and Burkitt's lymphoma previously reported has been confirmed by a second study. Analysis of additional data from a second study of 33 Ghanaian patients with African Burkitt's lymphoma and 54 Ghanaian controls matched for age and ethnic origin showed that 39.4% of cases, but only 14.8% of controls, had HLA-DR7 (p < 0.01). The relative risk of 3.7 is similar to that observed in the earlier study (3.3). Combining the earlier and present studies, analysis of clinical data from 94 patients with Burkitt's lymphoma and 116 controls shows the relative risk of Burkitt's lymphoma among individuals with HLA-DR7 was 3.4 (p < 0.001). There was an increased relative risk of the disease associated with HLA-DR7 in: (i) patients under 10 years of age; and (ii) patients with advanced stages of disease (Stage III or IV). However, comparison of relative risks by sequential analysis of 2 × 2 tables showed that these differences by age and stage were not statistically significant.Item Malaria in urban and rural areas of southern Ghana: A survey of parasitaemia, antibodies, and antimalarial practices.(Bulletin of the World Health Organization, 1984) Gardiner, C.; Biggar, R.J.; Collins, W.E.; Nkrumah, F.K.A comparative cross-sectional survey was undertaken in two populations, urban and rural, in southern Ghana to assess the impact of urbanization on the prevalence of malaria parasitaemia and antibodies. At the same time, a survey of antimalarial practices was conducted on sample populations in the two communities. The results showed a low parasite rate (16%) and correspondingly low titres of malaria antibodies in a significant proportion of the urban community, particularly in children less than 10 years old. This was associated with widespread use in the urban community of antimalaria drugs, particularly chloroquine, as prophylaxis. The parasite rate in the rural community was 22%, and 97% of the sample population over 1 year of age had antibodies against Plasmodium falciparum. These results demonstrate that a substantial proportion of urban children are growing up with little exposure to malaria, even in a region considered endemic for malaria. The implications of these findings are discussed.Item Epstein-barr virus RNA in burkitt tumor tissue.(Cell, 1979) Dambaugh, T.; Nkrumah, F.K.; Biggar, R.J.; Kieff, E.Analysis of the viral RNA in four Burkitt tumor biopsies indicates that tumor tissue contains RNA homologous to at least 3–6% of the DNA of Epstein-Barr virus (EBV). Most of these RNA species accumulate in the polyadenylated RNA fraction of Burkitt tumor tissue. Two approaches have been used to determine the location within the EBV genome of the DNA sequences which encode stable RNA in two Burkitt tumor biopsies, F and S, which contain 6–10 copies per cell of at least 80% of the EBV genome. With the first approach, 32P-EBV DNA homologous to polyadenylated or nonpolyadenylated RNAs from the F, S or R tumors was hybridized to blots of fragments of EBV DNA. With the second approach, polyadenylated or nonpolyadenylated RNAs from the F or S tumors were hybridized to separated, labeled fragments of EBV DNA in solution. The results indicate that first, most of the viral RNA in Burkitt tumor tissue is encoded by approximately 20% of the Hsu I D fragment, 20% of the Eco RI A/Hsu I A double-cut fragment and 3% of the Hsu I B fragment of EBV DNA; second, an abundant RNA species in tumor tissue is homologous to the “additional DNA” present in the W91 and Jijoye/HR-I Burkitt tumor isolates of EBV and absent in the B95-8 virus, an isolate of EBV from outside the Burkitt endemic region; and third, there is little or no homology to other regions of the EBV genome.Item Type-I HTLV antibody in urban and rural Ghana, West Africa.(International Journal of Cancer, 1984) Biggar, R.J.; Saxinger, C.; Gardiner, C.; Collins, W.E.; Levine, P.H.; Clark, J.W.; Nkrumah, F.K.; Blattner, W.A.The prevalence of antibodies against the newly described human T-cell lymphoma virus, type I (HTLV-I) in two communities in Ghana, West Africa, is described. There was no difference by community (urban, 3.6% and rural, 4.0%). Prevalence increased with age, being 5.9% among persons greater than 10 years old, but did not differ by sex. There was no difference when data were analysed by housing status or crowding. Non-confirmed reactions in the assay system were frequent and correlated with both prevalence and titer of antibody against malaria. Possible explanations include vector-borne transmission like that of malaria, but the relationship is more probably due to a polyclonal stimulation of B cells, enhancing the potential for detecting reactivity in the assay. Because assay systems vary and because most laboratories do not routinely use a confirmation assay, results presented by different groups must be interpreted cautiously.Item Humoral immune response to Epstein-Barr virus antigens and immunoglobulin allotypes in African Burkitt lymphoma patients.(International Journal of Cancer, 1984) Biggar, R.J.; Pandey, J.P.; Henle, W.; Nkrumah, F.K.; Levine, P.H.We examined whether any immunoglobulin allotype was associated with an elevated risk of Burkitt's lymphoma because of the association between Burkitt's lymphoma and abnormalities of the immunoglobulin-bearing chromosomes. The distribution of Gm and Km phenotypes among 56 Burkitt lymphoma cases and 25 age-matched controls was unremarkable. Burkitt's lymphoma has also been associated with the Epstein-Barr virus, and we therefore sought a relationship between immunoglobulin allotypes and antibody response to the component antigens of the Epstein-Barr virus, VCA, EA-D, EA-R and EBNA. No immunoglobulin phenotype was associated with higher antibody titers against any antigen, but there appeared to be an interactive effect in which persons homozygous at both Gm and Km loci had elevated titers against several of the component antigens of the Epstein-Barr virus. This is the first report of an interactive effect between immunoglobulin allotypes and viruses. Antibody titers against antigens of the Epstein-Barr virus were higher in cases than in controls, as expected. However, female cases had significantly higher titers than male cases, a finding similar to that previously reported in healthy persons.
